We host many events throughout the year to raise money for cancer research, including our annual Cure Bowl. We're only able to host such wonderful events because of volunteers like you!

RELEASE AND WAIVER OF LIABILITY

Please Read Carefully

I, the undersigned ("Volunteer"), desire to volunteer for Orlando Sports Foundation, Inc. ("OSF") and for others engaging in activities related to the Cure Bowl (collectively, the "Activities"). In consideration for the opportunity to participate in such Activities, Volunteer hereby freely, voluntarily, and without duress executes this Release and Waiver of Liability ("Release") and acknowledges, understands and agrees to the following:

1. Release and Waiver. Volunteer does hereby release and forever discharge and hold harmless each and all of Orlando Sports Foundation, Inc., Delta Airlines and other Cure Bowl sponsors, their respective officers, directors, employees and agents and their respective successors licensees, affiliates and assigns (collectively, the "Released Parties") from and with respect to any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the Activities.
Volunteer understands that this Release discharges each and all of the Released Parties from any liability or claim that the Volunteer may have against any of the Released Parties with respect to any bodily injury, personal injury, illness, death, or property damage that may result from the Activities, whether or not caused by the negligence of any of the Released Parties. Volunteer also understands that each and all of the Released Parties do not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to, medical, health, accident or disability insurance in the event of injury or illness.

2. Medical Treatment. Volunteer does hereby release and forever discharge each and all of the Released Parties from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Activities.

3. Assumption of the Risk. Volunteer hereby expressly and specifically assumes the risk of injury or harm in the activities and releases each and all of the Released Parties from all liability therefrom including, but not limited to, any known hazardous activities whether or not hereinafter listed.
4. Insurance. Volunteer understands that the Released Parties do not carry or maintain medical, health, accident, or disability insurance for any Volunteer.

5. Use of Likeness, Etc. Volunteer does hereby grant and convey to the Released Parties (A) all rights, title, and interest in any and all photographic images and video or audio recordings made by the Released Parties during the Activities and (B) the right to use Volunteer's name, photograph, likeness, image, voice and biography in any media, including publications, advertising and publicity, in connection with the Volunteer's participation in the Activities.

6. No Self-Promotion. During or with reference to the activities, Volunteer agrees not to engage in self- promotion of Volunteer's business, personal or otherwise, including but not limited to, communications containing promotional products or services, solicitations, advertisements of or relating to such business.

7. No Felony/Misdemeanor. Volunteer has not been arrested or convicted of a felony or misdemeanor relating to the health and safety of children. Volunteer is not subject to a current protective order and does not have a protective order filed against someone else.

8. Other. Volunteer expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Florida and that this Release shall be governed by and interpreted in accordance with the laws of the State of Florida. Volunteer agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

9. Validity. This Release is valid and binding from and after the date of execution hereof by Volunteer and shall remain valid and binding for any and all Activities engaged in by Volunteer until a written revocation thereof is signed and delivered by the Volunteer to OSF.
